Affidavit of Citizenship

This form is for the collection of DHS or other U.S. citizenship/nationality documentation. It is available for use by students who are unable to present their original documents in person to the Evanston Office of Graduate Financial Aid. Complete and submit this form, and attach your SIGNED citizenship documentation requested on your CAESAR To Do List, as well as a copy of your government-issued ID. Please be sure to include a copy of both sides of each document. By completing this form, you are certifying that the attached documentation is a true, exact, and complete copy of the original documents.
